Fee Intent Form Please fill out this form if you or the organization that you represent intend on creating or removing a new student lock-in fee, or increasing or decreasing an existing lock-in fee. Please factor the surcharge and fees into the amount you are requesting. Students will vote on the total and be informed that 25% and 1%, of that total is for the surcharge/fees. The RTA is 25% and 1% AS Admin fee of the total fee after the surcharge is added to the base fee. For example, if your program requires $1.00 per student per quarter base fee, the total fee amount on the petition and ballot will be $1.26 (Please refer to past supplement page on the election website for additional examples). Additionally, all new fees and increases to existing fees will be required to carry an administrative assessment, to be determined by the Office of Budget and Planning, between 3-8% (will probably be 7%).
